Hall of Fame

2011 HALL OF FAME AWARDS

Special Olympics Utah’s Hall of Fame awards are presented to the following five categories:

1. Athlete

2. Program Builder

3. LETR Flame of Hope

4. Project Unify

5. Outstanding Organization

Winners will be honored at the 2011 Hall of Fame induction ceremony on November 17. Inductees must be present.

Procedures:

· Any individual may nominate more than one candidate in each category

· Self nominations are acceptable

· Additional information may be requested from the nominator and/or nominee

· Nominations may be submitted by teams, coaches, staff, board members, volunteers, etc

· Nomination forms must be returned by September 20, 2011 to Rochelle Deeter: deeter@sout.org or fax: 801-363-1524.
